Refer to the examples for the structure, but do not limit yourself to the constructs identified there. Your table should reflect the theory that you have chosen as the focus for your intervention, and should include the performance objective, the personal determinants (these will be the theoretical constructs of the theory you have identified), and the change objective related to both (what needs to change related to the personal determinant for the individual to achieve the performance objective).
